Never Events often occur when the correct procedure has

not been followed, following or during treatment. They can

arise over a vast range of areas however most commonly

occur in the following situations:

   - Wrong Site Surgery

   - Scalding of Patients

   - Wrong implant/prosthesis

   - Overdose of medication

   - Retained foreign object post procedure

Due to the serious nature of the failings by the Defendant, should a Never Event occur, they are

largely indefensible and are becoming increasingly common. Between the period of April 2018 and

the 31st March 2019, over 496 serious incidents met the Never Event criteria.
